Referrals to Coliber

Please note that whilst we do accept referrals from medical specialists and psychologists, we also require it to be accompanied by detailed information from a General Practitioner for all patients. This is important as the doctor will require background health information, in order to inform safe prescribing.

Where possible referrals need to contain the following information:

• General medical history, risk factors, medications, and psychiatric history.
• BMI, weight, height, heart auscultation and blood pressure readings.
• Copies of any recent blood test results and ECG.
